Dental attrition
Dental attrition is a type of tooth wear caused by tooth-to-tooth contact, resulting in loss of tooth tissue. Tooth wear is a physiological process and is commonly seen as a normal part of aging. Advanced and excessive wear and tooth surface loss can be defined as pathological in nature. The pathological wear of the tooth surface can be caused by bruxism, which is clenching and grinding of the teeth.
If the attrition is severe, the enamel can be completely worn away leaving underlying dentin exposed, resulting in an increased risk of dental caries and dentin hypersensitivity. It is best to identify pathological attrition at an early stage to prevent unnecessary loss of tooth structure as enamel does not regenerate.

TOOTH CAVITIES
Tooth is exposed to food/drinks containing sugar and starches. Over time, the enamel is destroyed, forming a cavity. The signs and symptoms of cavities vary, depending on their extent and location. When a cavity is just beginning, you may not have any pain and discomfort and can repair with a filling. If cavities aren’t treated, they get larger and affect deeper layers of your teeth. They can lead to severe toothache, infection and tooth loss [Root canal treatment / extraction]. Regular dental visits and good brushing and flossing habits are your best protection against cavities and tooth decay.
Tooth abrasion
Tooth abrasion is the loss of tooth structure by mechanical forces from a foreign element such as hard tooth brushes, tooth picks etc. The appearance is commonly described as V-shaped when caused by excessive pressure during tooth brushing. Sometimes the ridge is deep enough to see the pulp chamber within the tooth itself.
Treatment
Modification of oral hygiene habit (such as avoiding overzealous brushing, use of soft bristle toothbrush) is important to prevent further progression. Existing abrasion cavities can be restored by dental fillings.

ROOT CANAL TREATMENT
Root canal treatment, also known as endodontic treatment, is a dental procedure in which the diseased or damaged pulp (core) of a tooth is removed and the inside areas (the pulp chamber and root canals) are filled and sealed. Once the tooth is sufficiently cleaned out and as sterile as possible, the root canal is sealed off and later Capping (Crowns) should be done to prevent tooth fracture.
The decision to do RCT can be taken based on the following factors.

3. Clinical examination
A good digital x-ray will reveal if the cavity is involving the nerve, then RCT is recommended. In case of borderline scenario where the cavity is deep but not involving the nerve, a decision can be taken after completely removing the decay .If the nerve gets exposed then RCT is necessary. Remember the primary goal of attending to that tooth is to remove the decay from it. So it is best to take a decision on the spot after removing the decay. If RCT is needed to save the tooth then it is recommended.
We have a STANDARD PROTOCOL such as, if cavity which we start out as a filling but realize that it is deeper and we feel that RCT is needed- then the patient will be explained about the situation and the cost. If the patient agrees then we can start the RCT right away. Otherwise we can only give a temporary filling not a permanent filling.
